This includes Chronic Disease Management Plans (Care Plans), Team Care, Enhanced Primary Care Referrals, and Mental Health Care Plans.
Referrals for all Specialist and Allied Health care (physio, podiatry, optometry, dietician, etc) at local facilities and Local and Melbourne hospitals. Real professionals not just "wellness influencers".
Our Registered Nurse provides immunisations, Dressings, Blood Tests, and caring advice on a range of medical issues.
